BNNI (65) 1-21, doi:10.21608/bnni.2025.369242.101 Cellulite is a common disorder affecting subcutaneous tissue with changes of skin topography. Thighs, hips, and buttocks are the most commonly affected areas Phosphatidylcholine Mesotherapy is carried out to reduce the appearance of cellulite by intradermal injections through effect on local subcutaneous fats. Our aim was to evaluate the lipolytic effect of phosphtidylcholine mesotherapy in the treatment of cellulite in buttocks and upper thigh regions. Patients and Methods: Low caloric diet, exercise and PPC mesotherapeutic injection weekly for six weeks for group (1). Low caloric diet, exercise for group (2). Both groups were evaluated before and after the study for: Anthropometric parameters including weight, BMI, and circumferences (Waist circumference, Hip circumference and Waist Hip Ratio), cellulite grade, laboratory evaluation, digital cellulite photographs and musculoskeletal ultrasonic (MSUS) fat thickness measurement. Results: After the study two groups showed statistically significant reduction in anthropometric parameters, lipid profile and FBS (p <0.001 for each group) with more reduction in Group (1).A significant reduction in cellulite staging showed in Group 1 compared to Group 2 (p=0.006)with improvement in cellulite grades compared to baseline (p<0.001 for Group 1 and p=0.046 for Group 2).Reduction in ultrasonographic fat thickness measurement in buttocks and upper thigh were observed in Group (1) (p2<0.001* for both sides).Conclusion: PPC mesotherapy is safe and offers a non-surgical option for cellulite management.PPC leads to multidimential improvment with reduction of anthropometric parameters and cellulite grade. PPC mesotherapy also had a greater impact on reducing MSUS fat thickness with possitive changes in body fat reshaping.
